Malnutrition is often pictured as simple hunger, but the reality is far more complex. It is a condition where the body receives too little, too much, or the wrong balance of nutrients. Its effects ripple across every stage of life, from a baby in the womb to an elderly person, and they shape both physical health and long-term wellbeing. Understanding how malnutrition harms different age groups, and recognising that it can exist even in people who eat enough food, is the first step toward tackling one of the most persistent public health challenges of our time.

Why some groups are more vulnerable than others
Malnutrition does not affect everyone equally. Certain groups face a much higher risk because their bodies have greater nutritional demands or fewer reserves to fall back on. Children under five, pregnant and lactating women, adolescents, and the elderly sit at the top of this list. Micronutrient deficiencies take a particular toll on these vulnerable groups, driving illness and reducing quality of life.
The scale of the problem is significant. India carries one of the largest burdens of malnutrition in the world, and it has long been the leading risk factor for health loss among children under five and women of reproductive age. Poverty, food insecurity, poor sanitation, and limited healthcare access combine to push the most marginalised communities into the deepest nutritional gaps.
Children and the first 1,000 days
The period from conception to a child’s second birthday, often called the first 1,000 days, is the most critical window for nutrition. Damage done here is frequently irreversible. Undernutrition makes children far more vulnerable to disease and death, leaving them with stunted growth, low body weight, poor concentration, and delayed physical and mental development.
Pregnant women and the cycle that repeats
A mother’s nutrition directly shapes her child’s future. Poor nutrition among women of reproductive age has consequences that stretch across generations. When a mother is undernourished, she faces a higher risk of giving birth to an undernourished child, perpetuating the cycle of malnutrition. The risk rises further when the mother is an adolescent or when pregnancies are spaced too closely together.
One striking outcome is the “thin-fat” newborn. Babies born to mothers with chronic malnutrition and anaemia often have low birth weight, elevated body fat under the skin, and insulin resistance, which raises their risk of cardiometabolic disorders in adulthood. In other words, undernutrition before birth can set the stage for diabetes and heart disease decades later.
The elderly
Older adults are frequently overlooked in conversations about nutrition, yet they are highly susceptible. Reduced appetite, difficulty chewing, chronic illness, and social isolation all chip away at their intake. The result is weakened immunity, slower recovery from illness, and greater frailty. Like infants and pregnant women, the aged have increased requirements for nutrients such as zinc and are at greater risk of deficiency.
The physical toll of undernutrition
Undernutrition is what most people imagine when they think of malnutrition: a body deprived of the energy and protein it needs to function. Its physical signs are visible and measurable, and public health experts track them using three key indicators.
Stunting, wasting, and being underweight
Stunting refers to low height for age, a sign of chronic, long-term undernutrition. Wasting means low weight for height, indicating acute and often severe nutritional loss. Being underweight combines both, reflecting low weight for age. These conditions are widespread among Indian children. National survey data has shown that stunting in children under five fell from 48% in 2005-2006 to 38% in 2015-2016, but the figures remain high, and wasting actually rose over the same period.
Weakened immunity and frequent infection
One of the most dangerous consequences of undernutrition is its effect on the immune system. A body short on nutrients cannot mount a strong defence against infection. This creates a vicious cycle: malnutrition weakens immunity, infections then drain the body of nutrients and appetite, and the child or adult slides deeper into poor health. This connection helps explain why undernutrition is so closely linked to higher mortality, especially among young children. Tens of millions of Indian children live with weak immune systems, higher disease risk, and impaired cognitive development as a direct result.
Muscle wasting and physical weakness
When the body does not receive enough energy and protein, it begins to break down its own muscle tissue for fuel. This leads to muscle wasting, fatigue, and reduced physical strength. For adults, this translates into lower work capacity and productivity. For children, it means less energy to play, learn, and grow, which affects development at every level.
Overnutrition: when too much also harms
Malnutrition is not only about scarcity. Overnutrition, driven by excessive calorie intake and poor diet quality, is now a rapidly growing problem. As incomes rise and lifestyles change, diets have shifted toward refined grains, vegetable oils, sugary drinks, and ultra-processed foods, while physical activity has declined.
This nutrition transition has produced a sharp rise in obesity and its related diseases. Overnutrition increases the risk of diabetes, hypertension, and cardiovascular diseases. The concern is especially acute because of how the body type common in this region responds to excess weight. Research shows that Indians tend to develop metabolic syndrome, hypertension, and type 2 diabetes earlier and at lower body weights than many other populations, partly due to higher body fat and relatively lower muscle mass.
The double burden of malnutrition
A particularly difficult feature of the situation is the coexistence of too little and too much. India faces what experts call the double burden of malnutrition, where undernutrition and obesity exist within the same population, household, or even the same individual. It is now common to find an underweight or anaemic mother living in the same home as an overweight family member, or a single person who is both anaemic and overweight. This makes designing nutrition programmes genuinely complex, because solutions for one problem can worsen the other if not carefully balanced.
Hidden hunger: the deficiency you cannot see
Perhaps the most underestimated form of malnutrition is hidden hunger. This is the term used for chronic micronutrient deficiency, whose health impacts are not always acutely visible. A person can eat enough calories every day, feel reasonably full, and still be dangerously short of the vitamins and minerals the body needs to function. Because the symptoms are subtle and develop slowly, hidden hunger often goes unnoticed until real damage has been done.
The three most widespread deficiencies worldwide are iron, iodine, and vitamin A. Each carries distinct consequences. Iron deficiency leads to anaemia, causing fatigue and reduced capacity for work and learning. Iodine deficiency impairs thyroid function and brain development. Vitamin A deficiency damages vision and weakens immunity. Vitamin A and zinc deficiencies weaken the immune system, while lack of zinc impairs growth and can cause stunting, and iodine and iron deficits prevent children from reaching their full physical and intellectual potential.
Why hidden hunger matters so much
The reach of hidden hunger is enormous. It is estimated to affect more than two billion people globally, roughly one in three, and its effects on children during the first 1,000 days are especially severe. Crucially, these deficiencies often appear in children who are not classified as malnourished by the usual measures of stunting or wasting, which is exactly why the problem stays hidden.
Anaemia remains one of the most stubborn public health challenges, particularly for women and adolescent girls. National data confirms that anaemia and iron deficiency are highly prevalent across the vulnerable population, undermining health, learning, and the safety of pregnancy. Addressing hidden hunger therefore requires more than just filling stomachs; it demands dietary diversity, food fortification, and targeted supplementation programmes.
Connecting the threads
The effects of malnutrition are best understood as a connected web rather than separate problems. Undernutrition in a mother produces a low-birth-weight baby. That baby grows up with a higher risk of obesity and diabetes if the diet later swings toward excess calories. Hidden hunger quietly undermines immunity and learning throughout. And the elderly, often forgotten, face their own version of the same nutritional neglect. Each stage feeds into the next, which is why breaking the cycle early, especially during pregnancy and the first 1,000 days, offers the greatest return.
Recognising the full range of malnutrition’s effects, from visible wasting to invisible deficiency to the diseases of excess, allows for smarter, more humane responses. Nutrition is not simply about quantity. It is about the right balance, at the right time, for every person at every age.
